Enter First 3 characters of last name and first two of first name. If already in use then extend the first name to 3 characters etc.

There are no required fields for Meditech build, fill in the following:

*Provider Type-RF

*Last name, First name the title

*Company if listed

*Address

*City

*State

*Zip

*Phone and Fax number

*Direct address if listed on NPI site

*Abstract Svc

*NPI number
